Wedding booking form and terms for providing entertainment services

Thank you for choosing SNAG for your music entertainment. In order to help us plan for your function, at this early stage, please fill in as much information in the spaces provided & return this form completed to secure your booking with us. Any information you cannot provide now will be gathered when we contact you prior to your function date. By completing and returning this form to us you confirm your intentions to engage our services and accept our terms & conditions.
Please provide us with a running time schedule of your function ie. Guest arrival time/meals/speeches/ etc…please email a copy if available or inform your reception coordinator to provide us with a running schedule on the night.
Please nominate a song for your bridal dance from SNAG’s set list or provide us with a CD with the song of your choice. (Please note: we cannot guarantee that CD’s will always function correctly), to avoid disappointments, we suggest you email an mp3 file to us or mail us your CD prior to your function & we will down load it onto our MP3 system. If you have any other special songs to be played during the function that are not on our song list, please also forward these to us.
All CD’s will be returned to you.
Terms & Conditions:

1.  Your MC or the function manager should be responsible for ensuring timeliness of your function. We do not take responsibility if your function runs off time or for
     events that may occur that are beyond our control. We may need to make adjustments to playing times accordingly.
2.  We kindly ask for receipt of full payment before the commencement of the function
3.  We reserve the right to delay our performance at the function until payment is received as we believe it inappropriate to disturb anyone during or after your
     function to collect our fees. We ask that our policy regarding payment of fees is respected to avoid any disappointment.
4.  Please note we do not accept personal cheques. Bank cheques addressed to Snag C/O F.Tiatto or EFT is acceptable - Note: these payment methods attract a
     $60 process fee which is in addition to the service fee quoted above.
5.  If during the function you request extra performance time beyond the official conclusion time as shown above, this is charged at $150 per ½ hour. (Minimum 2
     hours notification is required before the end of the function).
6.  If your selected venue requires us to negotiate at least one flight of stairs or is a high rise building, $150 lugging fee is charged in addition to the service fee
     quoted. We ask that you please inform us appropriately.
7.  Please ensure that you check with the venue for the supply of a microphone & in house PA system for your speeches. If none is supplied then you are welcome
     to use our system, but please advise us prior to the function. If you require the use of our microphone, please advise us immediately before the beginning of
     your function.
8.  In the event that you change your venue, a re-quote of our fee will be necessary.
9.  Please note that the venue is responsible to ensure that sufficient adequate power is provided to cater for entertainment that requires use of a PA system &
     lighting. Our equipment uses 240volts and can draw up to approximately 20amps. We suggest two (15amp) power sources on separate circuits be provided
     (Please check with Venue). For private venues, please check that your power is sufficient to handle supply for all electrical needs. If you are unsure, we suggest
     that you seek advice from a registered electrician.
10. $450 cancellation fee applies if notice of cancellation is less than thirty (30) days prior to the function date specified above.
11. It is a requirement under our insurance contract not to allow patrons in the stage area. To ensure the safety of patrons, we ask that patrons do not enter the stage
     area at any time. Failure to comply will result in disruption or termination of our performance without penalty to Snag.
12. Any variation or non disclosure prior to the function date may result in additional costs or variation to our fee.
We kindly ask for the following:

· 1 meal each including beverages (each member) to ensure that we are present for the full duration of the function.
· 1 small table & chairs to seat each member
· Please ensure that parking is provided for us to avoid delaying our setup. Parking is required for 3 vehicles including one standard trailer.
· Minimum 5m wide x 2.5m deep area to setup. Setup or stage location must be safe from traffic & weather. (Please check with Venue)
